Established Home Care Agency in Prince William County is looking for an experienced Home Care Coordinator/Scheduler
The Scheduler is responsible for preparing and coordinating the daily/weekly/weekend schedules for all field staff on behalf of the Clients of IncrediCare Home Health Care. This includes completing the schedule, the coordination of visits, maintenance and upkeep of scheduling records and logs and the accurate and timely communication of scheduling changes between office and field staff.
Responsibilities for this position include:
- Responsible for the preparation of the daily and weekly schedule for caregiver and clinical staff
- Responsible for assigning staff to Clients based upon a variety of factors including capability, geographical coverage, availability and patient needs
- Responsible for rescheduling and reassignment due to call outs and other reasons
- Ensures all shifts are scheduled two weeks in advance and within 48 hours for new admissions
- Confirms all caregiver shifts in advance and reports potential gaps in care to Administrative team
- Responds to clients and staff calls regarding visit issues and other concerns
- Responsible for disciplinary action when warranted.
- Ensure visits and hours are accurately recorded.
- Participates in rotating Primary On-Call requirements to ensure staffing needs are met.
- Available to resolve staffing issues at any time.
- Completes weekly calls to all active clients and caregivers to ensure satisfaction and problem resolution, if needed
- Evaluates, monitors and enforces the quality of services delivered to ensure compliance with IncrediCare policies, procedures, standards and regulatory compliance requirements.
- Ensures field staff is informed of procedures/protocols for providing care in residential facility, to include documentation requirements
- Actively participate in recruiting, hiring and training field staff
- Represents and promotes the agency in a professional manner
- All other duties as assigned
QUALIFICATIONS
- Minimum of three years home care scheduling experience
- Exceptional customer service skills
- Strong verbal communication skills and telephone etiquette
- Team leadership skills
- Excellent organizational skills
- Excellent attention to detail
- Ability to prioritize in fast paced environment
- Proactive problem solving skill with strong focus on problem prevention
- Proficiency in Microsoft office suite
- Proficiency in EMR systems or able to learn quick
Job Type: Full-time
Pay: $62,400.00 - $68,000.00 per year
