We are seeking a dependable LPN to support client visits, assist with care plan oversight, and provide guidance to caregivers and families in a non-medical home care setting.

Key Responsibilities

• Conduct scheduled client visits

• Assist with client assessments and ongoing monitoring

• Support implementation and updates of care plans

• Provide education and guidance to family caregivers

• Communicate client status changes to clinical leadership

• Support CNA caregivers through observation and feedback

Qualifications

• Active LPN license in Virginia or compact state

• Valid driver’s license and reliable vehicle

• Current CPR certification

• Home care or community-based experience preferred
