Perform routine scheduled visits with clients to monitor care and oversee CNAs in the home.  Maintain detailed and relevant care plans.  Assist Marketing team when your expertise is needed.

Responsibilities
 

• Conduct Weekly and Bi-Weekly visits to Clients
• Maintain accurate and current care plans
• Coordinate Client needs with their PCP as needed
• Provide counsel and guidance to family members on managing conditions

 
Requirements:

• Active LPN or RN License
• Current CPR (American Heart Association) Certification
• Willingness to adhere to health and safety standards, including those outlined by the CDC in relation to COVID-19
• Access to reliable transportation
• At least One year Experience in Home Care Environment
